HOW TO APPLY

• Contact HMS and speak with our social worker.
• Arrange to visit and tour the school.
• Talk with your school district about your interest.
• Gather and submit your child’s records to HMS. This information should include the following:
  
  • Re-evaluation Report (RR)
  • Current IEP (Individual Education Program)
  • Medical summaries
  • If available—hearing, vision and psychological evaluations
